Freshman Advisor Visitation e-Form

Instructions 

The following form has been provided for your convenience to track visitations with your freshman advisees. Each time an advisee visits your office please fill out this form and print it. Please return a copy to the Office of Academic Success and Student Retention and feel free to keep a copy for your records. If you wish, you may fax this form to our office.

This initiative is being done to continue to support our freshman population and to ensure that they are taking the necessary steps to help them succeed. If you have any questions comments or concerns regarding the form please feel free to contact our office. Our information is provided below.

Procedures for use of form

1. Contact advisees who are on probation once per month.
2. After talking with the student, fill out the form summarizing the conference.
3. Submit the form electronically to the office of Academic Success and Student Retention (ASSR).
